چکیده

We report the case of a 73-year-old female patient with diabetic nephropathy and cholelithiasis. She was admitted to our hospital with right upper abdominal pain, nausea, and vomiting. The patient had visited an outpatient clinic with the same complaints 2 days earlier, and had been prescribed antibiotics empirically (two doses ofloxacin orally). Blood cultures taken before the start of antibiotic treatment in our hospital were negative. The patient was treated with parenteral ampicillin/sulbactam + ciprofloxacin empirically. The empiric antibiotic treatment was discontinued after 7 days. Elective cholecystectomy was performed after her general condition improved. An aerobic chocolate agar culture of the cholecystectomy material yielded Haemophilus influenzae type b. On postoperative day 3 the patient developed fever again. The fluids collected after cholecystectomy were evaluated microbiologically. H. influenzae type b was isolated from the samples and blood cultures. The patient was diagnosed with H. influenzae cholecystitis, and recovered after 10-day treatment with ampicillin/sulbactam + ciprofloxacin. The findings are discussed together with references for differential diagnosis. H. influenzae cholecystitis due to cholelithiasis, although rare, should be considered in elderly patients with a history of chronic diseases such as diabetes mellitus or nephropathy.

Characterization of an ampicillin-resistant Haemophilus influenzae type B.

A 28-year-old female in Denver was found in early 1974 to have frontal sinusitis, osteomyelitis, and bacteremia due to Haemophilus influenzae, type B. The minimal inhibitory concentration of ampicillin for this organism was 100 mug/ml and the minimal bactericidal concentration was >100 mug/ml. It was inhibited by chloramphenicol at 0.4 mug/ml. Further studies demonstrated that ampicillin and me...

Comparative in vitro and in vivo activity of temocillin (BRL 17421) and ampicillin against Haemophilus influenzae type b.

A total of 42 strains of Haemophilus influenzae type b isolated from pediatric patients were sensitive in vitro to temocillin (90% minimal inhibitory concentration = 0.25 microgram/ml). No difference in mean minimal inhibitory concentration between beta-lactamase producer (0.25 microgram/ml) and nonproducer (0.23 microgram/ml) strains was found. Various dosages of ampicillin or temocillin for t...

Iodometric detection of Haemophilus influenzae beta-lactamase: rapid presumptive test for ampicillin resistance.

Strains of Haemophilus influenzae type b sporadically isolated from clinical specimens are ampicillin resistant due to production of a beta-lactamase. This enzyme which inactivates ampicillin and penicillin G is not produced by ampicillin-susceptible strains. Various characteristics of beta-lactamase production and ampicillin resistance of three H. influenzae type b isolates were investigated. ...

Ampicillin-resistant Haemophilus paraphrophilus laryngo-epiglottitis.

A case of life-threatening laryngo-epiglottitis is reported, caused by ampicillin-resistant Haemophilus paraphrophilus. Clinicians and microbiologists should be aware of a beta-lactamase-mediated resistance among Haemophilus species other than H. influenzae.

Beta-lactamase effect on ampicillin treatment of Haemophilus influenzae B bacteremia and meningitis in infant rats.

Intraperitoneal injections of 250 mg of ampicillin per kg every 6 h for 30 h sterilized the blood and cerebrospinal fluid of infant rats infected with either a beta-lactamase-containing strain of Haemophilus influenzae type b or a strain lacking the enzyme. However, a single injection of 100 mg/kg sterilized the blood and cerebrospinal fluid of significantly fewer of those rats infected with th...
